Why would I choose your practice?


You deserve the highest levels of expertise, experience, and personal attention from a physical therapist and physical therapy clinic. If you need more information, feel free to give us a call or just stop by the office to make sure you're 100% comfortable with our practice.




Do I need a doctor’s referral?


For most insurances, a therapy referral is not needed in Illinois. We will contact your insurance to verify the full benefit details. We will be able to submit claims to your insurance and will remain in close contact with your physician during therapy.




What types of health insurance do you accept?


We are contracted with or accept the following plans: most major commercial insurances, auto insurance, federal plans (ie Medicare and Tricare), cost-sharing programs and a self-pay rate. As a courtesy to you, we will verify your benefits prior to therapy. As an in-network provider for Aetna and Humana, we are contracted through an affiliate insurance network - Midwest Rehabilitation Group.




What can I expect at the first visit?


Your first visit will include a thorough history and examination by the physical therapist. Based upon these findings, a coordinated treatment plan will be suggested including detailed home exercise instruction and precaution review. The findings from your first visit will largely drive the initial strategies for the treatment of your condition while in the clinic.




Who will be providing my treatment?


You might have had experiences at other facilities where your sessions may have been split between multiple therapists or unlicensed aides may have participated in your care. Instead, we make things very simple. Your entire course of care, from evaluation to all parts of treatment, will be provided by Greg Given PT, MHS, SCS, ATC, CAFS.




What if I don't have insurance?


We do offer discounted self-pay rates for those patients who do not have insurance. Even without insurance, your investment in improving your health is money well spent and may save you from more expensive diagnostics tests or surgery. Physical therapy has also been proven to be an effective way of reducing dependence on medication for pain control.




Do you offer telemedicine services?


In response to the Covid19 crisis, we developed a telemedicine option for both new and current patients. We are currently utilizing doxy.me as a HIPAA compliant platform.




Will I receive home instructions?


We firmly believe that you should take an active part in your own recovery. At your first visit, we will discuss precautions and contraindications for your condition. In addition, we will provide your choice of printed exercises or electronic access including video demonstrations. Your home exercises will be modified through the course of your treatment.




Will my physician receive updates on my therapy?


Absolutely. Your physician will receive a copy of your evaluation summary shortly after your first visit. We will provide another update just prior to your next physician recheck or every 30 days - whichever comes first.




What do I need to bring to the first visit?


We would request that you bring in the following: completed new patient paperwork, a copy of any diagnostic reports, a physical therapy referral/prescription if you have one, a list of your medications, a picture ID and your insurance card. We would request that you bring or wear loose comfortable clothing so that we can assess your injury fully.
